In order to best understand your individual disorder, we need you to send us some personal information before your first visit. Seattle’s Elite Physical Therapy would like to make this process as quick and painless as possible. For your convenience, you can complete our intake forms online (no printing necessary!). When you submit your paperwork, they will be sent directly to your physical therapist for your initial visit. If you are unable to complete the forms before you scheduled appointment, you may show up for your appointment 30 minutes prior to your first visit and fill out the forms at the clinic. What to bring to your first visit:
- Your physician referral (prescription for therapy), if applicable. WA is a direct access state, which means most insurances cover physical therapy services without a physician referral. Medicare and VA require a referral.
- Your insurance card or claim number information, and a picture ID
- Loose, comfortable clothing and comfortable shoes (you may change before your visit, if needed)
Insurance Benefits for Physical Therapy
We know how challenging it is to navigate health insurance benefits and we are here to help. We accept all insurances, as well as , Auto Claims, HSA, FSA, and cash-based payments. Each insurance company offers many plans to their members, therefore your individual healthcare coverage is going to be specific to you and your plan. Most insurance plans offer a Member Services contact number that you can call and ask specific questions regarding your specific health insurance coverage, and we recommend you contact your plan provider to identify:
-How many visits a year are available under my plan?
-Do you need a referral for physical therapy?
-What requirements need to be met, in order for your insurance to cover the cost of your visit?
-Do you have a deductible to meet before insurance covers a portion of your visit?
-If there will be only a copay/co-insurance payment, what will your cost be per visit?
We do our best to verify your insurance benefits before you arrive, to allow you to understand any estimated out-of-pocket expenses and any visit limitations before you start treatment which is why we ask that you complete and submit your paperwork in advance to your first appointment. However, we always recommend you contact your insurance plan directly to understand your physical therapy benefits and coverage because regardless of your insurance coverage, the cost of the service provided to you is ultimately your responsibility.
If your insurance provider is out-of-network with us, that only means that we don’t have a specific, pre-negotiated contract with them and you will be able to pay our self-pay rates for your services. Fortunately, many insurance companies offer out-of-network benefits which often cover a significant portion of your treatment costs, although the percentage may differ from plan to plan. We will provide you with a superbill receipt to submit to your insurance for reimbursement.

Anthem, Blue Cross Blue Shield, Regence, Premera, Cigna, Lifewise, Medicare, First Choice, Kaiser Permanente (PPO), TriWest, and Motor Vehicle Accident (PIP claims).
We are Out of Network with the following insurance, but patients may submit their claims:
